Since the early 1990's, the Ohio State Bar Association, Ohio State Medical Association, Ohio Hospice and Palliative Care Organization, and other intrested groups have worked together to provide Ohioans with legally valid, universally recognizable, and easy-to-use Living Will, Health Care Power of Attorney, and organ donation directives ("health care advance directives"). Financial powers are usually delegated separately in a financial Power of Attorney. [download form]

Like any legal document, these forms should be used with caution. They grant broad powers to the person named to act for you. When we draft a Power of Attorney for a client, we use these forms, but we discuss them with you to make sure you understand them and agree with them. For example, these are generally designed to facilitate termination of life support for those in a permanently unconcious state or terminal illness, within the rules provided by Ohio law. If that is not your wish, we would cross out certain portions of the form and add additional notes. [download form]
