Most of our written work is created specifically for each individual. But there are some frequently-used documents that lend themselves to forms. In fact, a few forms relevant to estate planning are found in the Ohio statutes or have been endorsed by state-wide organizations such as the Ohio State Bar Association and Ohio State Medical Associations.

Our emphasis in estate planning is usually on personalized drafting. But when a standard form has been created by an authoritative body for a common situation, then its use should be carefully considered. One reason to use such a form is ease of recognition, for example, by the health care provider in the case of a health care advance directive, or by a bank in the case of a financial power of attorney.

In all cases, however, use a form with caution. Think about how it applies to your facts. Read it all the way through. Consult us if you have any questions.
